OCR description
The covered entity (CE), 360 Physical Therapy, reported that the protected health information (PHI) of 520 individuals was stolen during a robbery at the office of its business associate. The PHI involved included claims and billing information. The CE notified HHS and the affected individuals. OCR provided technical assistance regarding the HIPAA Breach Notification Rule.
