OCR description
The covered entity (CE), Morgan Stanley Health Benefits and Insurance Plan, reported that a software application used by its business associate (BA) exposed the protected health information (PHI) of 2,442 individuals. The PHI involved included names, addresses, Social Security numbers, diagnoses, lab results, medications, and other treatment information. The CE notified HHS and the affected individuals. In response to the breach, the CE provided complimentary credit monitoring services and the CE and BA implemented additional administrative, technical, and security safeguards to better protect its PHI.
