OCR description
The covered entity (CE), Veterans Health Administration, reported that its business associate (BA) mailed appointment reminder letters containing the protected health information (PHI) of 2,380 individuals to the wrong recipients. The PHI involved included names, addresses, and appointment information. The CE notified HHS, affected individuals, and the media. In response to the breach, the CE and BA implemented additional administrative safeguards to protect PHI.
