OCR description
The covered entity (CE), Daleville Dental, reported that it experienced a breach incident that affected the protected health information (PHI) of 5,280 individuals. The PHI involved included names, other identifiers, medications, and treatment information. The CE notified HHS and affected individuals. In response to the breach, the CE issued media, substitute, and individual notices; implemented additional administrative, technical, and security safeguards.
