The covered entity (CE), Catholic Health Initiative Trinity Medical Center, reported that its business associate (BA) experienced a hacking incident that affected the protected health information (PHI) of 797 individuals. The PHI involved included clinical, demographic, and financial information. The CE notified HHS, the affected individuals, the media, and provided substitute notice.
