OCR description
The covered entity (CE), Sharp Health Plan, reported that its business associate (BA) experienced a cyber-attack that affected the protected health information (PHI) of 9,255 individuals. The PHI involved included names, dates of birth, Social Security numbers, health insurance information, claims and financial information, and other treatment information. The CE notified HHS, affected individuals, and the media. In response to the breach, the BA implemented new administrative and technical safeguards.
