OCR description
The covered entity (CE), Concentra Health Services, reported that its business associate (BA) experienced a hacking incident that affected the protected health information (PHI) of 3,998,163 individuals. The PHI involved included names, addresses, social security numbers, and medical and claims information. The CE notified HHS, affected individuals, the media, and provided substitute notice. In its mitigation efforts, the CE provided free credit monitoring and the CE and BA implemented security measure to protect PHI.
