OCR description
Hampton-Newport News Community Services Board, the covered entity (CE), reported that it was the victim of a cyber-attack that affected the protected health information (PHI) of 44,312 individuals. The PHI involved included names, addresses, dates of birth, social security and drivers’ license numbers, claims information, diagnoses/conditions, lab results, medications, and other treatment information. The CE notified HHS, affected individuals, the media, and posted substitute notice on its website. In response to the breach, the CE implemented additional administrative, technical, and security safeguards.
