OCR description
The business associate (BA), HORNE, LLP reported that it was the subject of a data breach that affected the protected health information (PHI) of 180,467individuals. The PHI involved included clinical and demographic information. The BA notified HHS, affected individuals, and the media. In response to the breach, the BA provided complimentary credit monitoring services to the affected individuals, and implemented additional administrative, technical, and security safeguards to better protect PHI.
