OCR description
The covered entity and business associate (CE/BA), C.R. Pharmacy Services dba CarePro, reported that it experienced a cyber-attack that affected the protected health information (PHI) of 151,499 individuals. The PHI involved included names, addresses, dates of birth, drivers’ license and social security numbers, claims and financial information, diagnoses, conditions, lab results, and medications. The CE/BA notified HHS, the affected individuals, and the media. In its mitigation efforts, the CE/BA provided complimentary credit monitoring services and implemented additional administrative, technical, and security safeguards.
