Walker, Inc. d/b/a Walker Therapeutic & Educational Programs

The covered entity, Walker, Inc., reported that an employee was the subject of an email phishing scheme that compromised the protected health information (PHI) of 846 individuals. The PHI involved included names, addresses, birthdates, diagnoses, lab results, and medications. The CE notified HHS, affected individuals, the media, and posted substitute notice on its website. In its mitigation efforts, the CE implemented additional administrative, technical, and security safeguards to better protect its sensitive data. All staff were retrained on email security precautions.
