OCR description
Regence BlueCross BlueShield of Oregon, the covered entity (CE), reported that letters containing the protected health information (PHI) of 856 individuals were mailed to the wrong recipients. The PHI involved included names and health insurance information. The CE notified HHS, affected individuals, and the media. In response to the breach the CE retrained workforce members and implemented additional administrative safeguards to better protect PHI.
