OCR description
Coastal Hospice & Palliative Care, the covered entity (CE), reported that it experienced a cybersecurity incident that affected the protected health information (PHI) of 29,100 individuals. The PHI involved included names, addresses, dates of birth, Social Security numbers, claims information, diagnoses, and health insurance and other treatment information. The CE notified HHS, the affected individuals, and the media. In its mitigation efforts, the CE implemented additional administrative, technical, and security safeguards to better protect its sensitive data and retrained staff.
