OCR description
The covered entity (CE), Kern Regional Center, reported that an employee emailed the protected health information (PHI) of 772 individuals to her personal email account. The PHI involved included names, addresses, dates of birth, and treatment information. The CE notified HHS, the affected individuals, and the media. In response to the breach, the CE implemented additional administrative safeguards and retrained its workforce.
