OCR description
The covered entity (CE), the Veterans Health Administration (VHA), reported that an employee of its vendor mailed the protected health information (PHI) of 46,677 individuals to incorrect recipients. The PHI involved included names and treatment information. The CE notified HHS, the affected individuals, and the media. In response to the breach, the CE implemented additional administrative, technical, and security safeguards. Staff were retrained on the requirement to protect and secure sensitive data.
