OCR description
The covered entity (CE), Health Alliance Medical Plans, reported that a subcontractor of its business associate (BA) experienced a cyber-attack that compromised the protected health information (PHI) of 6,900 individuals. The PHI involved included names, addresses, birthdates, and Social Security numbers. The CE notified HHS, affected individuals, and the media. In its mitigation efforts, the CE terminated its business relationship with its BA.
