OCR description
Kentucky Cabinet for Health and Family Services, reported that an employee of its business associate (BA) inadvertently mailed the protected health information (PHI) of 857 individuals to the wrong recipients. The PHI involved included names, identification numbers, claims and financial information, and other treatment information. The BA notified HHS, affected individuals and the media. In its mitigation efforts, the BA implemented additional administrative safeguards and quality assurance procedures to prevent this issue from reoccurring.
