OCR description
The covered entity (CE), Liberty Hospital, reported that it experienced a cyber-attack that affected the protected health information (PHI) of 264,541 individuals. The PHI involved included names, birthdates, diagnoses, lab results, medications, and other treatment information. The CE notified HHS, affected individuals, and the media. In response to the breach, the CE implemented improved technical and administrative safeguards to better protect its PHI. OCR provided technical assistance to the CE.
