OCR description
The business associate (BA), CBIZ Marks Paneth, reported that it experienced a cyber-attack that compromised the protected health information (PHI) of 5,562 individuals. The PHI involved included names, birthdates, addresses, telephone numbers, medical record numbers, and health insurance and other treatment information. The BA notified HHS, affected individuals, and the media. In its mitigation efforts, the BA implemented additional administrative safeguards.
