North Hill Needham, Inc., North Hill Home Health Care, Inc. and North Hill Employee Dental Plan

The covered entity (CE), North Hill Needham, North Hill Home Health Care, and North Hill Employee Dental Plan reported that it experienced a hacking attack that affected the protected health information (PHI) of 1,096 individuals. The impacted PHI included names, addresses, phone numbers, dates of birth, dates of death, Social Security and drivers’ license numbers, health insurance information, claims and financial information, medications, lab results, and other treatment information. The CE notified HHS, affected individuals, and the media. In response to the breach the CE implemented additional administrative, technical, and security safeguards. Staff were retrained on the requirement to protect and secure PHI.
