OCR description
The covered entity (CE), Lena Pope Home, Inc., reported that an employee was the target of an email phishing scheme that affected the protected health information (PHI) of 3,954 individuals. The PHI involved included names, dates of birth, other identifiers, and diagnoses/conditions. The CE notified HHS, affected individuals, and the media. In response to the breach, the CE implemented additional administrative, technical, and security safeguards. Staff were also retrained in its requirements to protect and secure PHI.
