OCR description
The covered entity (CE), Amin Radiology dba Citrus Diagnostic Center reported that it experienced a hacking incident that affected the protected health information (PHI) of 1,273 individuals. The PHI involved included clinical, demographic, and financial information. The CE notified HHS, affected individuals, and the media. In response to the breach, the CE implemented administrative, technical and security safeguards.
