Employee Benefits Corporation of America and Benefit Design Group, the covered entity (CE), reported that its business associate (BA) experienced a cyber-attack that compromised the protected health information (PHI) of 38,912 individuals. The PHI involved included names, dates of birth, Social Security numbers, addresses, claims and financial information, and other identifiers. The CE notified HHS, affected individuals, the media, and posted substitute notice on its website. In its mitigation efforts, the CE changed all administrative passwords and enhanced backup and recovery solutions.
