OCR description
The covered entity (CE), Dignity Health Welfare Benefits Plan, reported that an employee mailed the protected health information (PHI) of 744 individuals to the wrong recipients. The PHI involved included names and medications. The CE notified HHS and the affected individuals. In response to the breach, the CE implemented additional administrative safeguards including retraining staff to better protect PHI.
