OCR description
The covered entity (CE), Hospice of Huntington, reported that it experienced a cybersecurity incident during which certain impacted files containing protected health information (PHI) may have been subject to unauthorized access or acquisition. The breach affected approximately 9,013 individuals. The PHI involved demographic, financial, and treatment information. The CE notified HHS, the affected individuals, the media, and provided substitute notice. The CE took several corrective actions in response to the breach, including the implementation of several technical safeguards. Additionally, OCR provided technical assistance to the CE.
