OCR description
The covered entity (CE), Interventional Pain and Regenerative Medicine Management, reported that a former employee removed notebooks containing the protected health information (PHI) of 2,843 individuals from the office. The PHI involved included clinical and demographic information. In response to the breach, the CE implemented additional administrative, technical, and security safeguards, and retrained workforce members to better protect its PHI.
