OCR description
OrthopedicsNY, the covered entity (CE), reported that it experienced a cyber-attack that compromised the protected health information (PHI) for 656,086 individuals. The PHI involved included names, mailing addresses, dates of birth, Social Security and drivers’ license numbers, passport numbers, financial and health insurance information, diagnoses, conditions, lab results, medications, and other treatment information. The CE notified HHS, affected individuals, the media, and posted substitute notice on its website. In its mitigation efforts, the CE restored impacted servers and deployed Sentinel-One anti-virus on all computers.
