OCR description
The covered entity (CE), Northeast Orthopedics and Sports Medicine, PLLC, reported that it experienced a hacking incident that affected the protected health information (PHI) of 177,101 individuals. The PHI involved included names, dates of birth, Social Security and drivers’ license numbers, medical record information, health insurance information, diagnoses, and other treatment information. The CE notified HHS, affected individuals, and provided media notification. In response to the breach, the CE implemented additional technical and security safeguards. Unfortunately, the CE ceased operations and is no longer a covered entity.
