OCR description
Ada Technologies, the covered entity (CE), reported that it experienced a cybersecurity incident that affected the protected health information (PHI) of 2,398 individuals. The PHI involved included names, addresses, dates of birth, and social security numbers. The CE notified HHS and affected individuals. The CE strengthened its administrative and technical safeguards and OCR provided technical assistance regarding its health information privacy compliance obligations.
