OCR description
The covered entity (CE), Eastern Radiologists, Inc., reported that it experienced a hacking incident that affected the protected health information (PHI) of 886,746 individuals. The PHI involved included names, Social Security numbers, addresses, dates of birth, diagnoses/conditions, lab results, and health insurance information. The CE notified HHS, affected individuals, the media, and posted substitute notice on its website. In response to the breach, the CE provided complimentary credit monitoring services and implemented additional administrative, technical, and security safeguards to better protect its PHI.
