OCR description
The covered entity (CE), Highland Health Systems, reported that it was the subject of a data breach that affected the protected health information (PHI) of 83,543 individuals. The PHI involved included names, addresses, drivers’ license and social security numbers, birthdates, medications, and other treatment information. The CE notified HHS, affected individuals, and posted substitute notice on its website. In its mitigation efforts, the CE implemented additional administrative, technical, and security safeguards to better protect its PHI.
