OCR description
The covered entity (CE), DGA-Producer Pension & Health Plans, reported that a software application used by its business associate (BA) exposed the protected health information (PHI) of 4,211 individuals. The PHI involved included names, Social Security numbers, and addresses. The CE notified HHS, the affected individuals, and the media. In response to the breach, the CE provided complimentary credit monitoring services and the CE and BA implemented additional administrative, technical, and security safeguards to better protect PHI.
