OCR description
The covered entity (CE), Lake of the Woods County Social Services, reported that it was the subject of a ransomware attack that affected the protected health information (PHI) of 537 individuals. The PHI involved included names, addresses, dates of birth, drivers’ license and Social Security numbers, claims and financial information, diagnoses, lab results, medications and other treatment information. The CE notified HHS, the affected individuals, the media, and provided substitute notice.
