OCR description
The covered entity, Bay Surgical Specialists, reported that its business associate (BA) experienced a ransomware attack affecting the protected health information (PHI) of 1,505 individuals. The PHI involved included names, addresses, Social Security numbers, birthdates, and treatment information. The CE notified HHS, affected individuals, and the media and provided affected individuals with complimentary credit monitoring services.
