BlueCross BlueShield of Tennessee, Inc. and Volunteer State Health Plan, Inc. d/b/a BlueCare Plus Tennessee

The covered entity (CE), BlueCross BlueShield of Tennessee and Volunteer State Health Plan dba BlueCare Plus Tennessee, reported that it experienced a cyber-attack that compromised the protected health information (PHI) of 1,231 individuals. The PHI involved included names, dates of birth, addresses, claims and financial information, diagnoses, and other treatment information. The CE notified HHS, affected individuals, and the media. In its mitigation efforts, the CE implemented additional administrative and technical and security safeguards.
