UC San Diego Health Hillcrest- Hillcrest Medical Center

The covered entity (CE), UC San Diego Health Hillcrest-Hillcrest Medical Center, reported that several employees were the subjects of an email phishing scheme that compromised the protected health information (PHI) of 1,642 individuals. The PHI involved included names, addresses, email addresses, dates of birth, medical record numbers, health insurance information, treatment cost information, and clinical information. The CE notified HHS, affected individuals, and the media. In its mitigation efforts, the CE provided complimentary credit monitoring services and implemented additional administrative, technical, and security safeguards to better protect its PHI.
