OCR description
The covered entity (CE), Blue Ridge Pediatric & Adolescent Medicine, reported that it experienced a cyber-attack that affected the protected health information (PHI) of 39,282 individuals. The PHI involved included names, addresses, dates of birth, Social Security numbers, and medical treatment information. The CE notified HHS, affected individuals, the media, and provided substitute notice. In its mitigation efforts, the CE provided complimentary credit monitoring services and implemented new administrative and technical safeguards. Staff were retrained.
