OCR description
The covered entity (CE), Massachusetts Department of Developmental Services, reported that it stored the protected health information (PHI) of 500 individuals in an unsecure facility. The PHI involved included names, dates of birth, Social Security numbers, addresses, diagnoses, lab results, medications, and financial and claims information. The CE notified HHS, affected individuals, the media, and posted substitute notice on its website. In response to the breach, the CE implemented additional administrative and physical safeguards to better protect PHI.
