The covered entity (CE), A5 Pharmacy, reported that it experienced a security incident that affected the protected health information (PHI) of 3,000 individuals. The PHI involved included clinical and demographic information. The CE notified HHS, provided substitute and media notice. In response to the breach, the CE implemented additional administrative and technical safeguards.
