OCR description
The covered entity (CE), California Correctional Health Care Services, reported that one of its workforce members inadvertently sent an email containing the protected health information (PHI) of 1,348 individuals to an unintended recipient. The PHI involved included names, diagnoses, lab results, and other treatment information. The CE notified HHS, affected individuals and the media. In response to the breach, the CE implemented additional administrative, technical, and security safeguards, and retrained its workforce member to better protect PHI.
