OCR description
The business associate (BA), RxBenefits, reported that a workforce member mailed the protected health information (PHI) of 3,396 individuals to the wrong recipients. The PHI involved included names, addresses, health insurance information, and medication type. The BA notified HHS, affected individuals, the media, and provided substitute notice on its website. In response to the breach the BA retrained the workforce member on mail merge procedures and implemented quarterly privacy training to all staff. OCR provided technical assistance regarding the HIPAA Rules.
