OCR description
Mary H. Makhlouf, DMD, MS, PA, learned that it experienced a cybersecurity incident that affected the protected health information (PHI) of 2,010 patients of the CE. The PHI demographic, clinical, and financial information. After conducting a review of this entity, OCR determined that Mary H. Makhlouf, DMD, MS, PA is not a “covered entity” as defined by regulations, and therefore we have no jurisdiction to investigate further.
