OCR description
The covered entity (CE), Robert Peterson, DDS, reported that it experienced a breach when unauthorized individuals entered his office and accessed and, in some cases, stole the protected health information (PHI) of 6500 individuals. The PHI involved included names, Social Security numbers, addresses, drivers’ license numbers, dates of birth, financial and claims information, diagnoses, lab results, medications, and other treatment information. The CE notified HHS, affected individuals, the media, and posted substitute notice on its website. In response to the breach, the CE provided complimentary credit monitoring services and implemented additional administrative, technical, and security safeguards to better protect its PHI.
