OCR description
The covered entity (CE), UT Southwestern Medical Center, reported that its workforce members impermissibly disclosed the protected health information (PHI) of 2,045 individuals without the appropriate contractual agreements in place. The PHI involved included names, addresses, dates of birth, financial information, diagnoses, conditions, and other treatment information. The CE notified HHS, the affected individuals and the media. In response to the breach, the CE sanctioned the workforce members involved, implemented additional technical safeguards, and implemented additional HIPAA training.
