OCR description
The covered entity (CE), Florida Pediatric Associates, LLC, reported that its business associate (BA) experienced a ransomware incident that affected the protected health information (PHI) of 1,104 individuals. The PHI involved included names, Social Security numbers, addresses, dates of birth, diagnoses/conditions, and claims and other treatment information. The CE notified HHS, affected individuals, and the media. In response to the breach, the CE offered free credit monitoring to affected individuals and implemented additional administrative, technical, and security safeguards to better protect PHI.
