OCR description
Pratt MRI, the covered entity (CE), reported that an employee lost a USB flash drive that contained the protected health information (PHI) of approximately 4,265 individuals. The PHI involved included names, dates of birth, addresses, and treatment information. Pratt notified HHS, affected individuals, and the media. In its mitigation efforts, the CE implemented additional administrative safeguards to better protect PHI.
