OCR description
The covered entity (CE), Rutgers Robert Wood Johnson Medical School, reported that it experienced a breach incident that affected the protected health information (PHI) of 2,399 individuals. The PHI involved included names, medications, and treatment information. The CE notified HHS and affected individuals. The CE notified HHS, affected individuals, and the media. In response to the breach the CE implemented additional administrative, technical, and security safeguards.
