OCR description
The covered entity (CE), Zuckerberg San Francisco General Hospital and Trauma Center, reported that a logbook containing the protected health information (PHI) of 754 individuals was lost. The PHI involved included names, gender, dates of birth, addresses, lab results, and other health information. The CE notified HHS, affected individuals, and the media. As a result of OCR’s investigation, the CE discontinued using logbooks and uses its electronic health record.
