OCR description
The covered entity (CE), Island Ambulatory Surgery Center, reported that it experienced a cybersecurity incident that affected the protected health information (PHI) of 8,266 individuals. The PHI involved include names, addresses, health insurance information, dates of birth, Social Security numbers, drivers’ license numbers, diagnoses, and other treatment information. The CE notified HHS, the media and affected individuals. In response to the breach, the CE implemented additional technical and security safeguards.
