Catholic Medical Center

The covered entity (CE), Catholic Medical Center, reported that an employee of its business associate (BA) was the subject of an email phishing attack that compromised the protected health information (PHI) of 2,792 individuals. The PHI involved included names, addresses, dates of birth, drivers’ license information, diagnoses, Social Security numbers, and other treatment information. The CE notified HHS, the affected individuals, the media, and posted substitute notice on its website. In its mitigation efforts, the CE provided complimentary credit monitoring services.
